Pathway plans

Find out what a pathway plan is and how it can help you prepare for the future.

A pathway plan is a document that you make with your social worker or personal adviser. It includes your needs, views, any future goals and what support you will receive.

The areas it covers include:

  • accommodation
  • education
  • training and employment
  • your safety
  • finance
  • health
  • identity
  • family and social networks

We will speak with you about what support you may need depending on your circumstances. For example, if you are:

  • at university
  • on an apprenticeship
  • a young parent
  • leaving custody
  • separated migrant young person
  • have special educational needs

Your looked after child reviews will end when you turn 18.

But, we can offer you a pathway plan review by your Independent Reviewing Officer when you're 18 and over.
