Request your care record

We understand that care records are an important source of information for care experienced adults. You can ask for a copy of the personal information we process about you in your care records. Sometimes we may remove information about other people in your care record. If we remove information we will show you where and tell you why.

To request information about another adult, you will need to provide:

  • your full name
  • your email address
  • your telephone
  • your date of birth
  • proof of your identity

Proof of identity can include a valid passport, driving license or birth certificate. If you have a key worker you can also ask them to confirm your identity to us.

Completing our online form is the best way to make sure that you are giving us the right information. Please check the information we do not hold before making your request.
