A pathway plan is a document that you make with your social worker or personal adviser. It includes your needs, views, any future goals and what support you will receive.
The areas it covers include:
- accommodation
- education
- training and employment
- your safety
- finance
- health
- identity
- family and social networks
We will speak with you about what support you may need depending on your circumstances. For example, if you are:
- at university
- on an apprenticeship
- a young parent
- leaving custody
- separated migrant young person
- have special educational needs
Your looked after child reviews will end when you turn 18.
But, we can offer you a pathway plan review by your Independent Reviewing Officer when you're 18 and over.
