What a personal adviser is and how they can help you.

A personal adviser is someone who is there to help you move from care to being independent.

They can give you advice and support on many areas of life. They will help make a document that outlines your goals and how you plan to achieve them. We call this a pathway plan.

Once you turn 18, your personal adviser will be responsible for supporting you.

You can continue to get help and support from them until you are 25.

Remember that you are not alone. Your personal adviser is there to help you every step of the way.
